Postpartum recovery is often framed as a six-week process. In reality, the evidence shows that full physical recovery — particularly of the pelvic floor, abdominal wall, and hormonal systems — takes significantly longer. Understanding the actual timeline helps set realistic expectations and identify warning signs that require medical attention.
The first 24 to 48 hours
The immediate postpartum period involves dramatic physiological shifts. Within hours of placental delivery, oestrogen and progesterone levels drop precipitously.1 The uterus begins contracting to control bleeding from the placental site. These contractions, commonly called afterpains, can be intense — particularly in multiparous women — and typically last three to five days.2
Vaginal bleeding (lochia rubra) is heavy and bright red during this phase. The uterine fundus is palpable at approximately the level of the umbilicus. Perineal swelling, bruising, and soreness are common following vaginal delivery, whether or not there has been a tear or episiotomy.3
Week one — the acute recovery phase
The uterus involutes at approximately one centimetre per day. By day ten, it is typically no longer palpable abdominally.2 Lochia transitions from bright red (lochia rubra, days one to three) to pale brown (lochia serosa, from approximately day four), and gradually to a yellowish-white discharge (lochia alba).3 Total lochia duration averages four to six weeks, though this varies.
Perineal discomfort peaks in the first few days. For women who have sustained lacerations or had an episiotomy, the REEDA assessment (redness, oedema, ecchymosis, discharge, approximation) guides clinical monitoring of wound healing.4 First- and second-degree tears typically heal within two to three weeks. Third- and fourth-degree tears may require six to twelve weeks for full tissue recovery.5
Weeks two to six — the subacute phase
By two weeks postpartum, the uterus has returned within the pelvis. Cervical and vaginal tissues remain oedematous and are healing gradually; in non-breastfeeding women, vaginal rugae begin to reappear around week three, with full epithelial restoration at six to ten weeks. In breastfeeding women, low oestrogen levels delay this recovery.2
This is the period during which many women begin to feel physically improved, but significant healing is still underway beneath the surface. Core and pelvic floor tissues are recovering gradually. Hair loss (postpartum telogen effluvium) may begin, typically resolving within six to twelve months.1
For women recovering from caesarean section, the abdominal incision requires careful monitoring. Current evidence-based guidelines recommend four to six weeks of recovery with restrictions on weight lifting, sexual activity, and vigorous physical activity.6 The Enhanced Recovery After Surgery (ERAS) Society updated its postoperative caesarean care guidelines in 2025, recommending early mobilisation, multimodal analgesia, and early removal of urinary catheters to improve recovery outcomes.7
The six-week check — a checkpoint, not a finish line
The six- to eight-week postnatal check has long been part of postpartum care in the UK, and since 2020 has been considered an essential GP service. NICE NG194 recommends it should cover mental health, physical recovery, contraception, and pelvic health.8 However, a 2025 mixed-methods study found that women frequently reported variable and sometimes poor experiences of this check, with only 62% of women in one English cohort having documented evidence of receiving one.9
The six-week mark does not represent full recovery. It is a clinical checkpoint — a point at which major complications should be identified and ongoing recovery plans established. Framing it as an endpoint creates unrealistic expectations and may discourage women from seeking help for symptoms that persist beyond this window.
Beyond six weeks — the extended recovery
Hormonal stabilisation takes six to twelve months. In breastfeeding mothers, oestrogen suppression continues for as long as prolactin remains elevated, which can affect vaginal tissue, libido, and bone density.1 Pelvic floor muscle recovery is similarly protracted: a prospective cohort study of primiparous women found a 20% reduction in vaginal resting pressure and a 7.5% reduction in pelvic floor muscle strength from mid-pregnancy to twelve months postpartum, with vaginal delivery being a significant risk factor.10
A 2024 systematic review of delivery-related factors affecting postpartum recovery, covering studies with a minimum follow-up of six weeks, confirmed that mode of delivery, perineal trauma severity, and operative interventions all significantly influenced recovery trajectories well beyond the traditional six-week window.11
When recovery crosses into complication
Certain symptoms at any point in the postpartum period require prompt medical evaluation:
Lochia: Persistence of bright red bleeding beyond the first week, foul-smelling discharge, or passage of large clots (larger than a golf ball) may indicate uterine subinvolution or infection.2
Wound concerns: Increasing pain, redness, swelling, or discharge at a perineal repair or caesarean incision site may indicate wound infection or breakdown.4
Systemic signs: Fever above 38°C, persistent severe headache, visual disturbances, calf pain or swelling, chest pain, or difficulty breathing require urgent assessment to exclude infection, venous thromboembolism, or persistent pre-eclampsia.8
