THRESHOLD planners are structured self-tracking and documentation tools. They are not medical devices, and nothing in them constitutes medical advice, clinical diagnosis, or treatment of any kind.

This page explains what "clinically grounded" means in the context of THRESHOLD products, and what our planners are — and are not — designed to do.

What "clinically grounded" means

THRESHOLD planners are described as "clinically grounded". This means each planner is designed using published clinical research, medical guidelines, and peer-reviewed literature as the structural basis for its content — including symptom frameworks, tracking structures, appointment prep systems, and screening tools.

It does not mean that the planners are clinical tools, medical devices, or therapeutic products. It means the structures and frameworks within them reflect current clinical understanding of the conditions and experiences they address — and that those structures are traceable to named, verifiable sources.

When THRESHOLD references a clinical body or guideline, we mean it accurately and specifically. We do not use the word "clinical" as aesthetic language or to imply a level of authority we do not have.

What our planners
are not.

THRESHOLD planners are planning, documentation, and self-tracking tools. They are not any of the following:

  • ×Medical advice or clinical guidance of any kind
  • ×Diagnostic tools — they cannot diagnose any condition
  • ×Substitutes for professional medical, psychological, or psychiatric care
  • ×Treatment plans or therapeutic interventions
  • ×Endorsed by or affiliated with any medical institution, clinical body, or regulatory authority
  • ×Evidence that any specific intervention or approach is appropriate for your individual situation

Where a planner includes a validated clinical screening tool (such as the EPDS or PHQ-9), it is presented as a self-report reference tool only. Scores or outputs from these tools are not diagnoses. They are indicators that should be discussed with a qualified healthcare provider.

Clinical sources used.

The following bodies and publications inform the structure of THRESHOLD planners. This list is not exhaustive — specific sources are available on request.

ACOG (American College of Obstetricians and Gynecologists), AWHONN (Association of Women's Health, Obstetric and Neonatal Nurses), NICE (National Institute for Health and Care Excellence, UK), CHADD (Children and Adults with ADHD), DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, 5th Edition), Edinburgh Postnatal Depression Scale (EPDS), PHQ-9 (Patient Health Questionnaire), AAP (American Academy of Pediatrics), SMFM (Society for Maternal-Fetal Medicine), and published research from Dr. Russell Barkley (executive function and ADHD) and Dr. Aurelie Athan (matrescence).

Referencing these bodies does not constitute endorsement by them. THRESHOLD is an independent product. These sources inform our design; they are not affiliated with or responsible for our products.

Your responsibility.

If you are experiencing a medical condition, mental health concern, or health crisis of any kind, please consult a qualified healthcare provider. THRESHOLD planners are tools that can support self-organisation and clinical communication — they are not a substitute for professional assessment, diagnosis, or treatment.

Do not delay seeking medical care because of information or frameworks contained in a THRESHOLD planner. If a section of a planner suggests a symptom or pattern that concerns you, discuss it with a qualified provider — the planner's job is to help you articulate and document; the provider's job is to assess and respond.

THRESHOLD does not assume any liability for decisions made based on content within our planners or based on self-scoring of any included screening tools.

Mental health content.

Several THRESHOLD planners include mental health tracking and screening content — including adapted versions of validated screening tools such as the EPDS (Edinburgh Postnatal Depression Scale) and PHQ-9. These are included as self-report reference tools to support provider conversations, not as diagnostic instruments.

If you score in a range that suggests significant mental health concern on any included screening tool, please contact a qualified mental health provider or your GP. The planner is designed to help you identify and document what you are experiencing — assessing and responding to that experience is the work of a qualified professional.

No endorsements or affiliations.

THRESHOLD is an independent product. No medical institution, clinical body, pharmaceutical company, healthcare provider, or regulatory authority has endorsed, approved, reviewed, or is affiliated with THRESHOLD planners or their content.

References to clinical guidelines, published research, or named researchers reflect the sources we used in building the planners — not partnerships, endorsements, or official approvals of any kind.

Liability.

THRESHOLD (the brand and its operator) does not accept liability for decisions made by users based on content within our planners, including self-scoring of any screening tools, symptom frameworks, or clinical reference material included in the products.

Our planners are documentation and self-tracking tools. The responsibility for medical, psychological, and health-related decisions rests with the individual and their qualified healthcare providers.

We also welcome feedback if you find a section inaccurate, out of date, or inconsistent with current clinical guidance. Every substantive correction goes into the next build.

Why this matters

We use the word "clinical" precisely
because most products don't.

A significant portion of the wellness product market uses clinical-sounding language without clinical substance behind it. "Clinically inspired". "Evidence-based". "Backed by science". Often these phrases mean nothing specific and reference nothing verifiable.

THRESHOLD uses "clinically grounded" because every framework in our planners can be traced to a named source. We are specific about what that means and what it doesn't — because the people using our planners deserve to know exactly what they're working with.
